Pen Injectors for Injectable Programs: A Buyer's Guide

Buyer's guide · Updated October 2026 · 3 min read

Multi-dose pen injectors make self-administered injectable programs easier for patients, and they let a pharmacy or brand offer a more polished product than a vial and syringe. Pens differ in how they're filled, how they dial doses and how they're branded.

Here's what to compare.

Reusable vs disposable

  • Reusable pens take replaceable 3 mL cartridges. The patient keeps the pen and swaps cartridges, which costs less per fill over time.
  • Prefillable disposable pens are filled by the pharmacy and discarded when empty. They're simpler for patients and look more like commercial products.

Specifications to compare

  • Cartridge compatibility: most use standard 3 mL glass cartridges.
  • Dose dialing and increments, and whether the dose is locked or adjustable.
  • Pen needle compatibility; most accept standard pen needles.
  • Build quality, dose accuracy data and how the pen is regulated.
  • Color options and custom branding at volume.

Who fills them

Filling cartridges or prefilled pens with medication is compounding or manufacturing, governed by state pharmacy rules and, for sterile preparations, USP chapter 797. Pharmacies and licensed manufacturers fill pens; we supply the empty pens and components.

Patient experience

Patients notice how a pen feels: the dial's clicks, the force needed to inject and how clearly the dose window reads. If possible, get sample pens and let staff and a few patients try them before committing to volume.

Questions to ask your supplier

  • Which cartridges and pen needles is it compatible with?
  • What are the dose increments and maximum dose?
  • Is it reusable or disposable?
  • What are the minimums for custom colors or branding?
  • What's the lead time on reorders?

Planning inventory

Forecast pens by new patients per month (for reusable pens) or by fills per month (for disposable pens). Pens, cartridges and needles should be ordered together so one doesn't run out before the others.

Ordering tips

Order pens, cartridges and pen needles together so the components match, and confirm minimums for custom branding and lead times.
